Registration Procedure

General Requirements
  • First Time

    FIRST TIME Registration can only be made at a Qhazanah Healthcare's Branch Office.

  • Renewal

    Renewal and subsequent registration can be made by agencies registered and the Qhazanah Healthcare's Branch Office.

  • Payment Method
    (Payable to Barisan Sistem Sdn Bhd)
    • Bank Draft
    • Online Banking Transfer
    • Cash
  • Registration Fee
    • RM 190 Male
    • RM 200 Female
  • Refund
    • To qualify for a refund, the medical form must be within the 90-day validity from date of registration and had not been used at the stated clinic or x-ray.
    • Any refund on the registration fees above will be subjected to an administrative charge of RM 30.
    • Please refer to our FAQ section for more info.
    • Refunds will only be based on registration fees only - i.e. excluding any other related costs incured.

    Please ensure that the following documents are ready.

  • Passport
    • Original and Photocopy of the worker's Passport with special pass.
    • Photocopy only the first page & entry chop.
  • Medical

    Medical report from clinic of origin.

  • Immigration

    A copy of Jabatan Imigresen's letter of approval or visa.

  • Qhazanah Healthcare
    • Registration fees.
    • Completed Registration Form.
  • Health Screening

  • Passport

    If there is a change of passport, please attach a copy of the new passport (First Page).
